Job Description
Provider will also need DHMF privileges as this service will transition to DHMF in the next few months!
Rate Caps for Submission:
- Malpractice Must be included in rates.
- Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
- Hourly BR50 Per hour not OT
- Holiday Must be 1.5x the regular rates Per holiday hour no OT
- Nightly Call BR100 Per night call to hold the pager to include 2.5 hours gratis
- Holiday Nightly Call BR650 Per night call to hold the pager to include 2.5 hours gratis
- Call Back BR50 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
- Holiday Call Back BR25 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

Requirement description :

- Board Certified or Board Eligible Required
- CA Active license Required
- Residency Required Required
- BLS and DEA required during credentialing. CA Fluoroscopy required at submission!
- Malpractice Clean Preferred
