Job Description
We are seeking a dedicated Licensed Professional Counselor (LPC) to join our multidisciplinary healthcare team. In this role you will deliver highquality behavioral health support ensuring seamless care for our patients.
What Youll Do:
Therapeutic Interventions: Provide evidencebased therapy focusing on behavior modification coping strategies and emotional health. Assist patients in processing trauma and building resilience.
Integrated Care Collaboration: Collaborate with physicians care managers and primary care staff to create and maintain comprehensive care plans. Offer insights into patient needs and emerging behavioral health trends.
Patient and Community Support: Conduct screenings and assessments and guide patients and families in accessing community resources and referrals. Provide crisis intervention when needed.
Documentation and Confidentiality: Maintain accurate detailed records of patient care in the EMR ensuring confidentiality and adherence to professional standards.
Qualifications:
Masters degree in Counseling or related field.
Active LPC license in the state of Michigan.
Experience in behavioral health therapy and patient care.
Strong communication and collaboration skills.
