The Clinical Referral Liaison reflects the mission vision and values of NM adheres to the organizations Code of Ethics and Corporate Compliance Program and complies with all relevant policies procedures guidelines and all other regulatory and accreditation standards.
The Home Care and Hospice Liaison acts as a liaison between hospital staff physicians community resources vendors and the Agency. Establishes cooperative relationships with referral sources. Provides consultation and education to increase hospital and community awareness of the range and scope of agency services. Communicates effectively with all divisions/programs in the agency. This role may support additional projects perform administrative tasks and other duties as assigned.
Responsibilities:
- The Clinical Referral Liaison coordinates and manages the home health and hospice referral and admission process.
- Travels to and works as main contact for assigned hospitals facilities and providers.
- Assists with discharge planning at time of evaluation.
- Assists to evaluate medical information to determine appropriate criteria is met for acceptance of patients into home health or hospice and medical stability in conjunction with medical staff home care hospice and hospital policy.
- Serves as a contact person for the intake team internal and external referral team clients and facilities to enhance outcomes.
- Identifies potential patients for Home Health and Hospice.
- Educate staff/patient/patient representative on the Home Health or Hospice Program qualifications.
- Assists in determining qualifications for Home Health and Hospice Programs.
- Works with referral sources and intake teams to collaborate continuation of care through Home Health or Hospice Programs.
- Provides coverage at other hospitals outside of assigned targeted area as needed or assigned.
- Discusses admission requirements (i.e. skilled needs homebound status hospice criteria etc.) with patient/family/referral sources as needed.
- Maintains a positive professional demeanor and presents situations in a positive light in all interactions with referral sources.
- Reports changes in targeted hospitals possible effects and action plans for the changing situations to the Director or designee as appropriate.
- Effective time management and organizational skills.

Outreach and Marketing
- Maintains a thorough knowledge of facilities or Providers needs to identify and perform learning opportunities
- Attends and conducts marketing and public relations activities.
- Collects data and develops strategic plans for referral management to capture referral gaps decrease leakage to other agencies.
Qualifications :
Required:
- High School Diploma/GED
- Computer experience in a Windows environment or equivalent experience.
- Demonstrates good verbal and written communication and organization skills.
- Valid drivers license with an automobile that is insured in accordance with state requirements.
- Strong customer service skills and ability to keep good relationships with customers coworkers and other business partners.
Preferred:
- Bachelors Degree in marketing business administration or other related fields is preferred. An acceptable combination of experience and/or training may be considered in lieu of formal education.
- Four years of home health or hospice care experience.
- Minimum of one year of professional sales experience preferred
