Assignment Start Date: 1/13/2025
16 weeks Days 8hour shift 40 hours weekly with 32 hours guaranteed
The Estimated Weekly Total is based on working the listed number of hours per week and includes available stipend amounts. Please contact a recruiter for full details. RN Case Managers evaluate reevaluate and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.
- Develops a care plan in collaboration with the patients physician the patient and their family or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
- Coordinate the patients care plan through interdisciplinary communication and case conferences as appropriate. Advise other disciplines of discharges promptly.
- Provide ongoing assessment of the patients clinical functional and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patients physician and colleagues.
- Document evaluations treatment goals and objectives.
- Teach the patient and caregiver selfcare techniques as appropriate. Provide medication diet and other instructions as ordered by the physician and utilize opportunities for health counseling with patients and caregivers.
EXPECTATIONS:
- Develop a plan of care under the direction of the patients physician and in collaboration with the patient their family and/or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
- Coordinates the patients care plan through interdisciplinary communication and case conferences as appropriate. Advises other disciplines of discharges on a timely basis.
- Provides ongoing assessment of the patients clinical functional and emotional status to identify problems and/or progress toward desired goals.
- Initiates and applies interdisciplinary and therapeutic interventions using appropriate techniques; obtains appropriate MD approval. Needs time frame immediately within 24 hours
- Supervises ancillary personnel (HCA and Licensed Practical Nurses) and delegates responsibilities when required. Establishes a care plan for paraprofessional services and revises it as needed according to agency policy.
- Completes and submits accurate and relevant clinical notes and other required documentation promptly following expected agency standards state and federal regulatory compliance i.e. admission papers and OASIS; clinical notes; day sheets; status change; resumption of care; verbal orders/memos; payer source documentation including management of authorizations; discharge.
- Completes all clinical documentation on the laptop and in the home when appropriate and in a timely fashion. Transmits all information twice a day.
EXPERIENCE/EDUCATION/QUALIFICATION
Experience:
- Must have Medical/Surgical nursing and/or home health care experience.
Education:
- Must be a graduate of an accredited school of nursing.
Licenses Certifications and/or Registration:
- Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- BLS/BCLS ACLS
EMR System: EPIC experience is preferred
Shift: Day shift 8:00 am 4:30 pm Monday Friday w/rotating weekends
