The Role:
This position is for a Physical Therapist (PT) in a home health setting providing care to patients in the comfort of their own homes. The role involves delivering physical therapy services as prescribed by a physician to improve mobility reduce pain and enhance overall function. The physical therapist will be responsible for creating and implementing treatment plans conducting exercises and rehabilitation activities and documenting patient progress. This position offers flexibility with a hybrid schedule covering a designated service area. The role also includes working collaboratively with other healthcare professionals and participating in ongoing education and training to ensure the best possible patient outcomes.
Key Responsibilities:
- Delivering physical therapy services according to physicians plan of care.
- Treating patients to relieve pain restore function and enhance performance.
- Assisting patients with exercises gait training and prosthetic training.
- Maintaining clinical records and documentation in a timely manner.
- Participating in team meetings and inservice education programs to enhance skills and knowledge.
Qualifications:
MustHave:
- Licensure: Licensed Physical Therapist in California.
- Education: Doctorate or Masters degree in Physical Therapy.
- Experience: Minimum of one year of PT experience required.
- Certification: Current CPR certification.
- Other: Valid drivers license insured vehicle in good working order
Day to Day
Location Flexibility: The role is hybrid covering patients in the Monterey area and nearby regions as needed.
Patient Interaction: You will work with patients to improve mobility help with rehabilitation exercises and document progress.
Team Collaboration: Engage in meetings with the clinical team share insights and participate in ongoing education programs.
Challenges
Traveling within the designated coverage area.
Maintaining accurate clinical documentation in a timely manner balancing patient care and paperwork.
Adapting to individual patient needs based on a variety of medical conditions and recovery processes.
Reporting Structure
This role will report to the Clinical Manager or Director of Rehabilitation Services. They will be supervised by the leadership team while maintaining autonomy in their patient care and scheduling.
Team
The team consists of a multidisciplinary care team that includes other physical therapists nurses and care coordinators. The team is relatively small but highly collaborative ensuring seamless patient care. You will often work independently but also share insights and treatment plans during team meetings.
