Job Description

The Division of Disease Controls mission is to safeguard the health of New Yorkers through identification surveillance treatment control and prevention of infectious diseases which is achieved through varied and interrelated endeavors of its seven Bureaus. The Bureau of Hepatitis HIV and Sexually Transmitted Infections (BHHS) mission is to improve the lives of New Yorkers by ending transmission illness stigma and inequities related to viral hepatitis HIV and sexually transmitted infections. BHHS accomplishes its mission through testing initiatives; prevention care and treatment programming; epidemiology and surveillance; training and technical assistance; community engagement; social marketing; policy advocacy; and racial equity and social justice initiatives. BHHSs HIV Care and Treatment Program (CTP) is the recipient of federal grants for HRSAs Ryan White HIV/AIDS Program (RWHAP) Part A and HUDs Housing Opportunities for Persons With AIDS (HOPWA). CTP is comprised of five units: 1) Grants Management 2) Program Planning 3) Quality Management and Program Implementation 4) Research and Evaluation and 5) Housing Services Unit. The HIV Care and Treatment Program (CTP) within the Bureau of Hepatitis HIV and Sexually Transmitted Infections (BHHS) is requesting approval to post a grantfunded position for a Housing Community Specialist position (Civil Service Title: Community Coordinator). The selected candidate will be responsible for working closely with external partners bureau and intraagency partners and BHHS Subrecipient Agreement holder (Public Health Solutions) to support the development and implementation of RWHAP Part A grantrelated projects and support the coordination of conditions of award.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:
Serve as the main point of contact for a portfolio of contracts federally funded to provide housing supportive services to people with HIV.
Advise on the development of quality indicators and quality improvement activities for assigned service categories.
Collaborate with the Research and Evaluation Unit in CTP to evaluate the process of implementing services to provide realtime actionable feedback on programs to describe and disseminate key findings.
Partner with the Subrecipient Agreement holder (Public Health Solutions) to execute subcontracts and renewals and ensure program integrity and quality through joint monitoring.
Coordinate the distribution of resources and information among community stakeholders such as communitybased organizations providing housing services.
Coordinate housing services for people with HIV eligible for the HOPWA program by conducting a phonebased intake and linking to a funded program.
Assist with the development of the HIV Housing Advisory Committees (HHAC) agenda including organizing guest speakers program coordination and meeting facilitation.
Support a total of three city councilfunded contracts providing client engagement services across housing services programs.
Other ad hoc duties as assigned.

PREFERRED SKILLS:
Professional working knowledge of HIV populations most affected by HIV and how structural oppression affects vulnerability to poor HIV outcomes.
Exceptional relationshipbuilding and influencing skills.
Keen attention to detail and ability to handle multiple and diverse assignments and priorities efficiently and effectively.
Excellent written oral facilitation and interpersonal skills.
Proficient in Microsoft Word Excel PowerPoint Outlook Visio and Adobe Acrobat.

Qualifications :

1. A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
2. High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
3. Education and/or experience which is equivalent to 1 or 2 above. However all candidates must have at least one year of experience as described in 1 above.
