The Population Health Data Strategist at NC Medicaids QPHE team facilitates a transformative project enhancing data exchange between payors and providers for quality improvement.
Responsibilities include but arenot limited to:
Provide subject matter expertise on QPHE sproject to leverage NC HealthConnex for quality and population healthincluding project design the implementation of healthcare provider supportsand developing strategies for future phases of work.
Lead related strategic efforts related to thecollection and exchange of data to support care management programs andprograms that address social drivers of health
Serve as liaison between QPHE s PopulationHealth staff Healthy Opportunities Pilot staff and the core staff leading theNC HealthConnex project
Ensure technology and business partners haveformative information to plan and adapt their work in adherence to bestpractices current policies and stakeholder demands
