Job Description

3 months contract with a Local Authority

Job Summary:

  • The Homeless Casework Service was established to help applicants who are homeless or living in temporary accommodation to move into suitable and sustainable housing. The Resettlement team works collaboratively with partners to move households into suitable housing and improve housing sustainment.
  • Within the Homeless Casework Service the Resettlement Team will provide specialist housing and support pathways for single homeless applicants with more complex needs.
  • The Specialist Resettlement Housing Coordinator will provide specialist casework to single homeless applicants leaving institutions such as hospitals and prisons and with complex support needs.
  • You must have expert knowledge of welfare housing and homelessness policy legislation and practice as well as specialist knowledge surrounding the complex needs of single homeless applicants.
  • You should also have knowledge of safeguarding vulnerable adults.

Key Duties/Accountabilities (Sample):

  • To provide excellent customer service to homeless applicants and key partners.
  • To provide specialist homelessness casework to single applicants with complex support needs leaving institutions ensuring that all aspects of casework are carried out in accordance with legislation strategies policies and initiatives set out at the national regional and local level through effective and regular case work supervision advice guidance and support.
  • Collaborate with internal and external partners to improve outcomes for single homeless applicants with complex needs and housing needs.
  • Contribute to embedding safeguarding best practice across the wider Housing Advisory Service.
  • Contribute to the development of the Homeless Casework Service and wider Housing Advisory Service.
  • To ensure our duties under the Homelessness Reduction Act are met delivering a proactive person centred outcome focused and end to end case management approach.
  • To undertake comprehensive assessments that lead to focused and coproduced Personal Housing Plans (PHP) for each applicant.
  • To assess applicants under the prevention and relief duties issue S.184 final decision letters ensuring effective communication and management of expectations with customers.
  • As appropriate undertake the assessment and completion of highquality risk management plans for applicants utilising information from a range of stakeholders and the applicant themselves.
  • To minimise risk by identifying reporting and following up any safeguarding concerns.
  • Possess and maintain an ongoing indepth knowledge of relevant welfare and housing legislation and the Council s statutory housing duties policies and procedures.
  • Provide advice and guidance to applicants by responding effectively and efficiently to enquiries maintaining the highest standards of professionalism and customer care.
  • Create and maintain accurate GDPR compliant client records and notes of all casework activities and decisions through both manual and IT recording systems.
  • Coordinate the provision of services and support for applicants in line with the PHP liaising with other teams and both internal and external services.
  • Attend various multiagency meetings as required such as discharge planning meetings and MAPPA (multiagency public protection arrangement) meetings.

Ability and experience of consistently achieving performance objectives in the provision of specialist homelessness casework. Ability to contribute to the identification, dissemination and embedding of existing and emerging best practice within the team, and across the wider Housing Advisory Service. Ability to effectively communicate with a range of stakeholders, including applicants, members, partners, and so on. Significant knowledge of welfare, homelessness and housing policy, strategies, legislation, and case law, and its implementation within a statutory homelessness service. Significant knowledge of safeguarding best practice, including policy, strategies, legislation, and case law, and its implementation within a statutory homelessness service.
