Responsibilities of a Home Health LPN
- Administer skilled nursing care in accordance with the physicians instructions and patients care plan
- Monitor patients condition and assess home environment
- Engage with patients families providing appropriate education and information regarding care
- Document clinical notes and maintain accurate health records
- Collaborate with interdisciplinary team to coordinate personalized care strategies
Key Qualifications for the Home Health LPN Role
- Valid LPN license in the state of PA
- Experience in EMR Other EMR TruChart EMR
- Certifications inany ofAHA ACLS or AHA BLS
- Previous home health or clinical experience is favorable
- Strong communication and interpersonal skills
- Ability to work independently and manage time effectively
