PDY 1 residency program post graduate PGY1 3 years of experience / PGY2 Residency they do not have to have any experience

JOB DESCRIPTION:
Medication review: Reviewing patient medication charts to identify potential risks and clarify information
Educating patients: Informing patients about their medications including dosage side effects and potential interactions
Monitoring patient progress: Keeping track of patient outcomes and ensuring they dont experience negative side effects
Advising healthcare professionals: Providing drug information and education to other healthcare professionals
Collaborating with healthcare teams: Working with doctors nurses and other healthcare professionals to ensure optimal patient care
Developing guidelines: Collaborating with the infection control team to develop tools and guidelines for the safe and effective use of antibiotics
Assessing pharmaceutical treatments: Evaluating the results of pharmaceutical treatments
Creating drug plans: Developing unique drug plans for patients
Maintaining medication profiles: Keeping medication profiles for patients including a registry of poisons and controlled drugs
