- MUST HAVE CVOR EXPERIENCE
- MUST HAVE EXPERIENCE WITH OPEN HEARTS AND VASCULAR CASES
- MUST HAVE EXPERIENCE WITH CABG/VALVES AND VASCULAR
- RESPONSE TIME IS 30 MIN
- BLS
MUST INCLUDE THE HOME ADDRESS & SSN ON THE SUBMISSIONS TRAVELERS MUST BE GREATER THAN 50 MILES FROM THE FACILITY
