Job Description
:$ 3531 per week
Duration: 13 weeks
Start Date: 1/20/2025
Responsibilities: Assessing planning organizing and participating in rehabilitative programs
Purpose: To improve patient mobility relieve pain increase strength and correct disabling conditions resulting from disease or injury
Requirement: Gaining confidence and cooperation from patients their families and other healthcare providers
Location: San Jose/South Bay/Santa Clara County
Requirements: BLS (AHA) 2 years of home health experience CA PT Certification
EMR System: HCHB
Shift: 5 x 8 Days
Weekend Requirements: Rotating Weekends every 67 weeks.
