Job Description

Position Name: Director of Case Management Leadership
Location: Lima OH 45804
Start Date: ASAP
Employment Type: Fulltime Travel Contract Temporary
Contract Length: 13 weeks Accepting Local and Travel Candidates
Pay Rate: $3800 weekly gross pay (Nontaxable travel/housing and M&I stipend available for qualified/eligible travelers)

Unit: Hospital Clinical Setting
Shifts Available: Days
Shift Timings: 8 Hours; Days 8:00 am 5:00 pm
Schedule: 40 hours/week guaranteed 5 shifts per week (MondayFriday with weekend and oncall as needed)

Position Requirements

  • Registered Nurse Experience: 5 years Required
  • Case Management/Discharge Planning: Minimum of 5 years Required
  • Hospital Clinical Role: Minimum of 3 years Required
  • Supervisory/Management Experience: Highly preferred
  • Flexibility: Must float to all units within skill level Required

Education License and Certification Requirements

  • BSN or Equivalent: Required
  • MSN or Equivalent: Highly preferred
  • RN License: State of Ohio or Compact Required
  • BLS Certification: American Heart Association (AHA) Required

Job Description

  • Functioning within the Health Systems mission values objectives and policies and procedures under the general direction of the Chief Nursing Officer Functioning within the Health Systems mission values objectives and policies and procedures the Director of Case Management provides overall direction for the clinical care coordination program which includes the coordination negotiation and procurement of care of patients to achieve quality and cost effective patient outcomes throughout the continuum of care of the patient which includes all health care settings from acute inpatient treatment through follow up in the office chronic disease management and preventative care.
  • This involves working collaboratively with interdisciplinary staff internal and external to the organization.
  • The Director has twentyfour (24) hour accountability for the management of human fiscal and physical/material resources of the Case Management department and is responsible for budgeting quality control and oversight of those who provide case management and discharge planning services.
  • The Director ensures that initiatives are in place to assure that quality cost containment patient and physician satisfaction and associate engagement goals are met facilitates the performance improvement process to improve care transitions reduce readmissions improve throughput and maintain high levels of professional practice; and integrates the findings into interdisciplinary hospital activities to effect change.
  • The Director collaborates with other disciplines in case review to identify opportunities for improving patient care outcomes reduction of future risk and enhancing customer satisfaction.
  • The Director provides resource and consultation to senior leadership regarding care coordination and regulatory standards. The Director promotes innovation team building and processes that enhance work environment.
  • This position utilizes and disseminates research and best practices recommendations to enhance evidencebased practice and autonomous decision making.
  • Upon approval the Director implements monitors and measures both clinical and financial success.
  • Responsibilities also include serving as a resource to staff and interpreting philosophy goals standards policies and procedures.
  • The Director collaborates with other facility leaders to create and implement an operational plan that is in alignment with facilitys strategic plan.
