Job Description
As a Diabetes Educator you will play a pivotal role in improving the health outcomes of patients by providing education support and evidencebased interventions for diabetes management. This role involves developing personalized care plans teaching selfmanagement skills and promoting a holistic understanding of diabetes to empower patients in managing their condition effectively. You will work closely with interdisciplinary teams to ensure integrated care delivery for individuals with diabetes.
Qualifications :
- Minimum of a Bachelors degree in Nursing Nutrition or a related healthcare field.
- Certification as a Diabetes Educator (CDE) is required.
- A minimum of 5 years of experience in diabetes education preferably in clinical or community health settings.
- Indepth understanding of diabetes management including current treatment protocols and technologies (e.g. insulin pumps continuous glucose monitors).
- Excellent communication counselling and education skills with the ability to present complex information clearly.
- Proficiency in tailoring educational interventions to meet diverse patient needs including cultural and socioeconomic considerations.
- Experience working with culturally diverse populations and adapting education to various health literacy levels.
- Proven ability to work effectively within interdisciplinary teams contributing to integrated care delivery.
