Job Description
The Primary Health Team Leader is responsible for overseeing the delivery of primary healthcare services within the organisation. This role involves leading a multidisciplinary team ensuring highquality patient care and promoting health equity in the community. The Team Leader will drive program development enhance team performance and collaborate with stakeholders to address the diverse health needs of the population.
Key Responsibilities:
- Team Leadership: Supervise and mentor primary health staff fostering a collaborative environment that promotes professional growth and effective communication.
- Program Management: Develop implement and evaluate primary health programs and services ensuring they are evidencebased and responsive to community health needs.
- Patient Advocacy: Serve as a key point of contact for patients advocating for their healthcare needs and ensuring they receive comprehensive coordinated care.
- Quality Improvement: Monitor and assess the quality of care provided using data and feedback to identify areas for improvement and implement necessary changes.
- Collaboration: Work closely with healthcare providers community organizations and stakeholders to enhance service delivery and expand access to care.
- Education and Training: Provide ongoing training and support to team members on best practices health promotion and patient engagement strategies.
Qualifications :
You are a dedicated healthcare professional with a passion for leading teams and improving community health outcomes. With strong communication and interpersonal skills you excel at fostering collaboration among diverse team members and stakeholders. Your experience in primary healthcare helps you with the ability to develop and implement effective programs tailored to patient needs.
Qualifications:
- Education: Bachelors degree in nursing Public Health or a related field; masters degree preferred.
- Experience: Minimum of 5 years of experience in primary healthcare with prior leadership or supervisory experience strongly preferred.
- Strong leadership and teambuilding abilities.
- Excellent communication and interpersonal skills.
- Knowledge of primary healthcare practices and regulations.
