Job Description
- Collaborates with multidisciplinary team members to assess inform educate and continually improve effectiveness and productivity of clinical and business programs.
- Anticipates and identifies complex clinical and business processes and ambulatory issues monitors and measures the risk factors. Communicates risk factors to informatics leadership and stakeholders. Manages projects to ensure the implementation of continuous quality control of process and deliverables.
- Works with clinical staff to implement streamlined and efficient workflow changes (documentation EMR tools etc.).
- Participates in planning and provides support for population health programs including but not limited to patientcentered medical home transitions of care health maintenance/prevention and chronic disease programs including those focused on diabetes COPD hypertension etc.
- Conducts data analysis to identify trends and patterns related to care outcomes. Develops action plans to address negative findings.
Qualifications :
- Bachelors degree in nursing. Masters degree in Nursing Informatics in process informatics or related field required.
- Clinical licensure in the State of Maryland or eligibility for licensure due to Compact state agreements is required.
- Three years of clinical experience required. Two years working with Epic required. Prefer ambulatory clinical experience.
