The RN Case Manager reports directly to a Clinical Supervisor and is responsible for care coordination of clinical staff patients and their families/caregivers and physicians or other care providers. Responsible for admissions recertification assessments transfers/resumptions of care and discharge assessments including supporting documentation.
Duties and Responsibilities
- Ensures direct supervision of CNAs LPNs and RNs in the field. This involves training and reeducation of staff on clinical skills/procedures when necessary.
- Develops an individualized plan of care for each client.
- Provides support guidance and education to patients and their families/caregivers to assist patients in their progress toward their goals.
- Completes all clinical documentation in a timely and accurate manner and ensures all documentation meets the requirements of agency policies and state regulations.
- Complies with and participates in continuing quality improvement by reporting incidents injuries and infections.
Qualifications :
- Current state license as a Registered Nurse BSN preferred.
- Minimum of one years of nursing experience required (two years preferred).
- Demonstrate leadership capabilities with strong supervisory and interpersonal skills.
- Detail oriented flexible good organizational and time management skills.
- Current CPR card and Current health certificate as applicable.
- Current state drivers license and automobile insurance.
Additional Information :
The salary range for this position is $70000$75000 annually. You are also eligible for a monthly auto and cell phone allowance totaling $4200.00 annually.
