Job Description

The RN Case Manager reports directly to a Clinical Supervisor and is responsible for care coordination of clinical staff patients and their families/caregivers and physicians or other care providers. Responsible for admissions recertification assessments transfers/resumptions of care and discharge assessments including supporting documentation. 

Duties and Responsibilities   

  • Ensures direct supervision of CNAs LPNs and RNs in the field. This involves training and reeducation of staff on clinical skills/procedures when necessary.
  • Develops an individualized plan of care for each client.
  • Provides support guidance and education to patients and their families/caregivers to assist patients in their progress toward their goals.
  • Completes all clinical documentation in a timely and accurate manner and ensures all documentation meets the requirements of agency policies and state regulations.
  • Complies with and participates in continuing quality improvement by reporting incidents injuries and infections.

Qualifications :

  • Current state license as a Registered Nurse BSN preferred.
  • Minimum of one years of nursing experience required (two years preferred).
  • Demonstrate leadership capabilities with strong supervisory and interpersonal skills.
  • Detail oriented flexible good organizational and time management skills.
  • Current CPR card and Current health certificate as applicable.
  • Current state drivers license and automobile insurance.


Additional Information :

The salary range for this position is $70000$75000 annually. You are also eligible for a monthly auto and cell phone allowance totaling $4200.00 annually.
