Job Description
- In conjunction with the Occupational Therapist / Physiotherapist and other members of the multidisciplinary team develops implements and evaluates individual programs
- Provides therapy sessions as instructed including balancing and strengthening exercises for seniors at risk of falling
- Attends client/patient related meetings to report on assessment findings and discusses plans for interventions
- Implements treatment plans based on assessment client/patient input and in conjunction with the client/patients integrated care plan
- Maintains accurate and complete clinical records on client/patients accordingly to the established procedure and ensures the confidentiality of these records
Qualifications :
- Certificate or Diploma in the Occupational Therapist Assistant or Physiotherapy Assistant Program from a recognized community college program or school registered with the Ministry of Training Colleges and Universities
- Minimum 1 year experience
- Demonstrated ability to work independently and as a member of a multidisciplinary team
- Proficiency in the use of technology (MS Office proprietary software)
- Current police check for work with the vulnerable sector
- Valid G drivers license third party insurance coverage must be maintained at $1000000.00 and must have a reliable vehicle
