Job Description

This is a unique opportunity to be part of a tightknit collaborative team from across the health sector that will deliver the next generation of palliative care in Mississauga providing holistic proactive seamless care and support to patients families and caregivers enhancing quality of life comfort dignity and security in patients end of life journey. As an integral member of the interdisciplinary care team the Physiotherapist (PT) Palliative Care works in collaboration with the care team client and family integrating a palliative approach. Specifically the PT is responsible for conducting assessments developing and delivering treatment plans focused on achieving the goals of care including mobility and function symptom management and delivering education. Care will be provided both inperson and through virtual consultations to maximize our ability to provide services in the community.

  • Works under the supervision of the Program Director
  • Collaborates with the interdisciplinary team to create personalized care plans based on the patients unique needs and goals of care.
  • Participates in clinical huddles and case conferences with team members such as physicians nurses social workers speech therapists and occupational therapists
  • Conducts physiotherapy assessment consultation and planned interventions for patients living with a lifelimiting illness
  • Engages in skilled compassionate communication with patients families and caregivers regarding mobility function and endoflife care goals
  • Educates and mentors patients family and caregivers on strategies to maintain safe mobility and function in the home setting
  • Maintains accurate and timely documentation of patient assessments treatment plans and outcomes ensuring adherence to ethical and legal standards

Qualifications :

  • Masters degree in Occupational Therapy
  • Registered in good standing with the College of Physiotherapists of Ontario
  • 35 years of experience working as an Physiotherapist preferred
  • Successful completion of any of the following is considered an asset:
    • Learning Essentials Approaches to Palliative and End of Life Care (LEAP); or
    • Fundamentals of Palliative Care
    • The Comprehensive Advanced Palliative Care Education (CAPCE)
    • Experience working in a community setting
  • Valid Ontario drivers license and a reliable vehicle
  • Ability to work autonomously and as part of an integrated health care team with an emphasis on collaborative care planning
  • Expertise in managing complex physical function and mobility issues in palliative care an asset
  • Proficiency in conducting assessments interpreting findings and formulating evidencebased treatment plans.
  • Familiarity with assistive devices adaptive equipment and environmental modifications
  • Excellent communication and interpersonal skills
  • Strong clinical skills and a passion for improving the quality of life for patients and their families
  • Effective problem solving and critical thinking abilities
  • Proficiency in computer software programs (e.g. Microsoft Office) and virtual communication platforms (e.g. Microsoft Teams Zoom Go To Meeting)
