Job Description
You will be responsible for providing highquality nursing care to patients in the community. You have clinical skills and extensive knowledge of wound ostomy and continence care management for a wide range of patients with chronic diseases.
- Provide patientfocused safe holistic nursing care and work collaboratively with other members of our team
- Ensure that nursing care provided to our patients is based on current and best practices to achieve successful outcomes
- Continually assess anticipate identify and respond to new and changing needs of our patients
- Travel between patients homes within a specific geographical region
Qualifications :
- Minimum of twoyear nursing experience (Community Nursing experience is ideal but not required)
- Currently registered and in good standing with the College of Nurses of Ontario (CNO)
- Must have completed Nurses Specialized in Wound Ostomy and Continence Canada (NSWOC) course
- Ability to work independently in managing your own caseload
- Proficiency in English both verbal and written required (if you can speak additional languages terrific)
- Valid Ontario drivers license and reliable vehicle with willingness to travel
- Basic proficiency in Word Excel and ability to learn software
