Officially we title this job an Admissions Coordinator but in other centers it may be called an Admissions Rep or Intake Coordinator Marketing Representative or Liaison.
What you will do in this role:
You will be responsible for daily census building activities of one of our Skilled Nursing Facilities (SNF) in Waukegan. This role is a combination of outbound marketing activities and inside admissions duties. You will spend 3050% of your time out of the center meeting with referral sources and doing bedside visits in the hospitals.
- Your number one goal is to achieve our budgeted census in this Skilled Nursing center.
- To do that you will need to create & engage in outbound business getting activities. These may be cold calls hosted events (like CE presentations) LunchnLearns at a variety of locations or even centerbased events meant to increase tours to the center. Youll need to be outgoing & organized.
- You should be self directed in your efforts and have the ability to manage relevant CRM metrics to ensure measurable results are attained. Example: Within the first 6 months we would expect you to increase our number of sources by 15% and increase our overall referrals by 25%.
- This position will not have any direct reports but will be intertwined with administration our central admissions partners & our regional marketing team.
After you drive in the referrals now you need to transition them into an admission.

When you are a patient the transition from hospital to SNF is stressful. As an Admissions Coordinator you will help to organize multidisciplinary services so the patient/resident can safely transition into our center. One moment you may be analyzing the cost of meds the next could be touring a family member and the next is ensuring your intake packet is correctly signed.
- You will need to have strong organizational and time management skills to be successful in this role. Its a busy day and you will need to be on the run.

Qualifications :
What we need from you:
- Prior experience as an admissions coordinator/director in a Skilled Nursing Facility is a must.
- Strong relationships with existing referral sources so you can immediately capitalize on & bring them with you. (You will be expected to rapidly expand our census.)
- Bilingual (Spanish) is ideal but not required. This center has targeted Spanish speaking programs. So it will be helpful when interfacing with families.
- Experience with Point Click Care (PCC) and a CRM is ideal
- Working knowledge of federal & state regulations along with reimbursement & insurance verification experience (Managed Care Medicare & private insurances)
- Associates or Bachelors Degree in a relevant healthcare field. Our ideal candidate will have experience as a Discharge Planner Social Services nursing or Admissions within the SNF setting. You will need this education and experience to regularly conduct patient assessments (bed side evals) to determine their fit for our environment.
