Job Description

Our client is actively seeking an experienced onsite MD or DO to support their busy post acute facility in Flagstaff AZ. This is a part time opportunity with a flexible schedule you choose the schedule.

Key Responsibilities
Patient Care Management:
- Provide primary care services to postacute patients including assessment diagnosis treatment and ongoing management of chronic conditions such as diabetes hypertension COPD heart disease and others.
- Develop and implement individualized care plans based on comprehensive assessments and patient needs.
- Monitor patients progress adjust treatment plans as necessary and provide proactive management to prevent complications.
- Provide primary care services to postacute patients including assessment diagnosis treatment and ongoing management of chronic conditions such as diabetes hypertension COPD heart disease and others.
Care Coordination:
- Collaborate with an interdisciplinary team of nurses therapists social workers and specialists to ensure holistic and coordinated care.
- Work closely with family members and caregivers to educate them about patient care plans chronic condition management and overall wellness.
- Manage transitions of care from the hospital to postacute care ensuring that patients receive continuity of care.

Chronic Disease Management:
- Focus on optimizing care for patients with multiple chronic conditions ensuring appropriate followup and preventive measures.
- Perform regular checkups and physical exams to monitor the progression of chronic diseases and mitigate potential risks.

Clinical Oversight & Documentation:
- Maintain accurate and uptodate medical records in compliance with regulatory standards.
- Ensure proper documentation for all assessments treatments and followups.
- Order and interpret diagnostic tests prescribe medications and provide clear instructions for patient care.

Onsite Availability & Patient Rounds:
- Conduct daily rounds to assess patient progress and collaborate with nursing staff on patient care needs.
- Provide onsite care at the facility in Flagstaff AZ.

Quality Improvement:
- Participate in quality improvement initiatives to enhance patient care and meet regulatory standards.
- Contribute to the development and implementation of best practices for chronic disease management and postacute care.

Requirements
- Medical Degree: MD or DO with completion of an accredited medical school program. Board certified or board eligible.
- Licensure: Active unrestricted medical license to practice in Arizona (or eligibility for licensure in AZ).
- Experience:
- At least 1 years of experience in a clinical setting (postacute care geriatrics or chronic disease management preferred).
- Experience in a postacute skilled nursing or rehabilitation setting is a plus.

- Skills:
- Strong clinical knowledge and experience in managing chronic conditions.
- Excellent communication skills and the ability to work collaboratively with an interdisciplinary team.
