Job Description
What youll do
As a Care Assistant specialising in hospital discharges you will be the compassionate face of our care team helping clients safely transition from hospital to home. You will provide essential support according to individualised care plans assisting with personal care needs such as bathing dressing managing incontinence and supporting with medication. Your role will also include practical tasks like preparing meals helping with housework and ensuring a safe living environment postdischarge.
