Our client is actively seeking an experienced Family or Adult Nurse Practitioner to support their busy post acute care facility. This is a flexible part time opportunity. 1 to 2 days per week you choose the schedule.
Key Responsibilities:
- Patient Care: Provide direct clinical care to residents including assessment diagnosis treatment and ongoing management of medical conditions.
- Comprehensive Assessments: Conduct physical exams review medical histories order and interpret diagnostic tests and develop treatment plans tailored to individual patient needs.
- Medication Management: Prescribe and manage medications including pain management antibiotics and other therapies as appropriate for postacute conditions.
- Care Coordination: Collaborate with attending physicians nurses physical therapists social workers and other healthcare providers to coordinate care plans and ensure effective transitions of care.
- Family Education and Support: Educate patients and families about treatment plans medications and lifestyle modifications to promote recovery and wellness.
- Clinical Documentation: Maintain accurate uptodate electronic medical records in compliance with facility and regulatory standards.
- Chronic Disease Management: Monitor and manage chronic conditions such as diabetes hypertension COPD heart disease and others commonly encountered in postacute care settings.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient outcomes and ensure compliance with healthcare standards and best practices.
- OnCall Responsibilities: Includes some afterhours or weekend coverage to address urgent medical issues.
Requirements
- Education: Master s degree in Nursing (Family Nurse Practitioner or Adult Nurse Practitioner program).
- Licensure: Current unrestricted license to practice as an FNP or ANP in the state of Arizona.
- Certifications: Board Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP).
- Experience: 0 5 years of experience providing direct patient care; new graduates encouraged to apply

- Strong clinical judgment and decisionmaking abilities.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of an interdisciplinary team.
- Proficiency in using electronic health records (EHR) and medical documentation systems.
