Job Description
Job Title: Physical Therapist Home Health
Location: Solano County California
Compensation: $3018.00 per week
Schedule: 40 hours per week MondayFriday with one weekend a month rotation
Contract Duration: 13 weeks
Start Date: ASAP
Responsibilities: Prioritize start of care visits supervise Physical Therapy Assistants next day reporting of start of care visits submit clinical notes and other documentation within 48 hours of visit maintain timely communication with supervisor and office.
Requirement: CA PT License and one year of Home Health experience
Territory: Sacramento CA (Expansion as needed)
Training: 3 full days (one in house two in the field)
Documentation System: Kinnser
Equipment Issued: iPad Verizon One Talk
Mileage Reimbursement: $0.65.5/mile (from first to last patient)
Parking and Toll Reimbursement: Paid
Company Car: Option available with a decreased bill rate
Bill Rate: $100
OT Rate: $5 paid after 30 points per week
Holiday Rate: $10
Weekend Rate: N/A
OnCall Rate: N/A
Call Back Rate: N/A
Productivity Requirement: 30 points per week
Visit Type Point Value: Start of Care 2 Recertification Resumption of Care OASIS Discharge Evaluation 1.5 Revisit NON OASIS DC 1
