**This is a Full Time position at 40 hours per week for our Lake Forest and Grayslake clinics. This role will assist in the navigation of our Gyn Onc patients and perform patient triaging. Schedule is Monday through Friday day shift.**
The Nurse Navigator reflects the mission vision and values of NM adheres to the organizations Code of Ethics and Corporate Compliance Program and complies with all relevant policies procedures guidelines and all other regulatory and accreditation standards.
Responsibilities:
- Coordinates and plans physician requested multidisciplinary conferences; tracks scheduled procedures and follows pathology results coordinates with pathologist and radiology the collection of slides and films related to a diagnosis and monitors outcomes of meetings.
- Interprets and guides patient through followup for laboratory radiology and pathology procedures.
- Follows up on patients with complicated or specialized needs.
- Works with patients in the rehabilitation recovery period following a cancer diagnosis.
- Serves as a resource for patients and their families while coordinating additional resources as requested.
- Coordinates patient referrals to insurance companies and doctors offices.
- Monitors quality of care physician and patient satisfaction through follow up.
- Teaches about cancer health concerns and provides pertinent lectures as needed.
- Educates patients concerning what can be expected prior to during and after a cancer diagnosis and treatment procedures.
- Evaluates the patients physical and emotional recovery after treatment utilizing appropriate resources as needed.
- Develops and implements survivorship care plans to patients with a cancer as they complete treatment.
- Identifies recommends and continues to establish supportive services to meet patients needs.
- Develops programs and provides staff and community education as required; maintains reference library.
- Pursue opportunities for public speaking to promote cancer care services at Northwestern Medicine Lake Forest Hospital.
Qualifications :
Required:
- Registered Nursing license issued by the State of Illinois.
- Bachelor of Science in Nursing.
- BLS through American Heart Association.
- Must demonstrate broad and thorough knowledge of the theories principles and practices of oncology care.
- Five years of oncology related work experience.
- Must be a self directed independent practitioner.
Preferred:
- Master of Science in Nursing
