Job Description

With funding from national state and local sources we deliver highquality affordable health care to nearly 10000 patients in over 26 area towns and are committed to providing integrated health Medical Dental Behavioral for the entire family. We serve ALL and no one is turned away for inability to pay. Our health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or healthrelated claims including medical malpractice claims for itself and its covered individuals.

Responsibilities
  • Conduct comprehensive patient assessments and physical examinations.
  • Diagnose and manage acute and chronic health conditions.
  • Develop and implement individualized patient care plans.
  • Prescribe medications and other treatments as necessary.
  • Provide preventive care including immunizations and health screenings.
  • Educate patients and families on health maintenance and disease prevention.
  • Collaborate with physicians and other healthcare professionals to ensure coordinated and comprehensive care.
  • Maintain accurate and uptodate patient records.
  • Participate in community outreach and health promotion activities.
  • Stay current with medical advancements and best practices.


Qualifications
  • Masters or Doctoral degree in Nursing from an accredited program.
  • Certification as a Family Nurse Practitioner (FNP) or equivalent.
  • Valid APRN license to practice in New Hampshire.
  • Strong clinical skills and knowledge of primary care.
  • Excellent communication and interpersonal skills.
  • Commitment to providing highquality patientcentered care.
  • Ability to work collaboratively in a teamoriented environment.
  • Experience in community health or underserved populations is a plus.
