Job Description
Overview
Key Responsibilities
- Coordinate the care of patients undergoing hematopoietic stem cell transplantation.
- Conduct comprehensive assessments of patients medical history and current health status.
- Serve as the primary point of contact for patients and their families throughout the transplant process.
- Provide education to patients and families regarding the transplant procedure risks and aftercare.
- Collaborate with a multidisciplinary team to develop individualized care plans.
- Manage pretransplant assessments and scheduling of necessary appointments and tests.
- Assist in obtaining financial authorizations and patient resources.
- Maintain accurate and detailed patient records and documentation.
- Monitor patient progress and address any complications or concerns during preand posttransplant phases.
- Facilitate communication between patients families and healthcare providers.
- Organize support services for patients including counseling and nutritional guidance.
- Participate in quality improvement initiatives within the transplant program.
- Stay updated on the latest research and guidelines in hematopoietic transplantation.
- Conduct followup appointments and assessments to ensure optimal recovery.
- Engage in continuous professional development and education related to transplant coordination.
Required Qualifications
- 3 years of RECENT (WITHIN THE LAST YEAR) acute care leadership experience within specialty (Would be open to 12 years; Also would be flexible on speaking with candidates that have leadership experience within the Transplant BMT Blood Cancer Hematology Oncology)
