Job Description

Overview

The Hematopoietic Transplant Coordinator plays a crucial role in the management of patients who are undergoing hematopoietic stem cell transplantation. This specialized position ensures that patients receive comprehensive care from diagnosis through treatment and followup. The coordinator acts as a liaison between various departments including medical nursing and social services to facilitate an effective transplant process. They educate patients and families about all aspects of the transplant journey from pretransplant assessments to posttransplant recovery ensuring they are wellinformed and prepared for each stage. Beyond clinical duties the coordinator is responsible for coordinating logistics managing patient records and collaborating with healthcare providers to optimize patient outcomes. This role requires a profound understanding of hematological conditions and the transplant process as well as exceptional organizational and interpersonal skills to ensure that each patients journey is as smooth and successful as possible.

Key Responsibilities

  • Coordinate the care of patients undergoing hematopoietic stem cell transplantation.
  • Conduct comprehensive assessments of patients medical history and current health status.
  • Serve as the primary point of contact for patients and their families throughout the transplant process.
  • Provide education to patients and families regarding the transplant procedure risks and aftercare.
  • Collaborate with a multidisciplinary team to develop individualized care plans.
  • Manage pretransplant assessments and scheduling of necessary appointments and tests.
  • Assist in obtaining financial authorizations and patient resources.
  • Maintain accurate and detailed patient records and documentation.
  • Monitor patient progress and address any complications or concerns during preand posttransplant phases.
  • Facilitate communication between patients families and healthcare providers.
  • Organize support services for patients including counseling and nutritional guidance.
  • Participate in quality improvement initiatives within the transplant program.
  • Stay updated on the latest research and guidelines in hematopoietic transplantation.
  • Conduct followup appointments and assessments to ensure optimal recovery.
  • Engage in continuous professional development and education related to transplant coordination.


Required Qualifications

  • 3 years of RECENT (WITHIN THE LAST YEAR) acute care leadership experience within specialty (Would be open to 12 years; Also would be flexible on speaking with candidates that have leadership experience within the Transplant BMT Blood Cancer Hematology Oncology)
