Job Title: Travel Social Worker Social Work
Job Description
Key Responsibilities
- Conduct assessments to determine the social emotional and financial needs of patients.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Provide counseling and support to patients and families facing various challenges.
- Facilitate communication between patients families and healthcare providers.
- Assist in resource allocation and connect patients with community services.
- Maintain accurate documentation of services provided and patient progress.
Requirements
- Licensure/Certification:
- Current California BBS Certification.
- Basic Life Support (BLS) certification from AHA.
- Experience: Minimum of 2 years of experience in social work.
- Active CA Licensure: Acceptable candidates include LCSWs ACSWs (MSWs) LMFTs and AMFTs (MFTs) registered within California.
Additional Information
- Schedule: Will alternate weekly between Tuesday to Friday and Wednesday to Saturday.
- For Travel Nurse RATE: $60 per hr / $50 for Local *50 mile radius .
