Key Responsibilities:
- Provide nursing care to patients in their homes ensuring their comfort and safety.
- Complete comprehensive assessments and develop individualized care plans.
- Document patient progress and manage care coordination effectively.
- Administer medications and treatments as prescribed maintaining accurate records.
- Communicate with physicians caregivers and healthcare team members to ensure continuity of care.
- Monitor patient conditions and report any changes in health status.
- Educate patients and their families on health management and wellness.
Minimum Qualifications:
- Three months of experience working at a MEDICARECERTIFIED home health agency.
- One year of experience in an acute care or skilled nursing setting.
- Six months of travel nursing experience preferred.
- Oasis experience required.
- Current RN licensure in California.
