Job Description
Position: Occupational Therapist (OT) Outpatient
Start Date: 1/6/2025
Duration: 13 weeks then perm. THIS IS FIRMLY TEMP TO PERM.
Location: Lebanon NH 03766
Shift Type/Shift Times/Hours/Day: Days 8
Shift Notes: 8hour shifts MondayFriday along with the occasional Saturday shift.
Weekend Requirements: TBD/Holiday rotation required
OnCall Requirements: None

Traveler Policy: Local Travelers (within 50 miles of facility) will not be considered for this need but are invited to apply for a perm position. Thank you! .
** TRAVELER MUST HAVE THEIR OWN TRANSPORTATION FOR THIS NEED! THIS IS A RURAL AREA WITH NO PUBLIC TRANSPORT.
** THIS NEED IS FIRMLY TEMP TO PERM. THE FACILITY WILL NOT CONSIDER TRAVELERS TO THIS NEED WHO WILL NOT GO PERM AFTER ASSIGNMENT.
** Requested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!! NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better).
The occupational therapist evaluates and treats assigned patients referred to Occupational Therapy by their physician. Provides supervision to COTA staff in the absence of staff OT.
- Responsible for the evaluation of all patients referred to Occupational Therapy.
- Practices Occupational Therapy consistent with the American Occupational Therapy Association Code of Ethics and Standards of Practice.
- Maintains a patient treatment schedule consistent with the needs of the department and the patient.
- Responsible for the preparation and submission of department reports ensuring the confidentiality of all records including Evaluation reports/summaries discharge reports/summaries and current progress notes on all patients in the assigned area.
- 3 years CURRENT OT Experience Required
- EPIC EMR experience REQUIRED
- Travel experience Preferred
- SHIFT ACCOMMODATIONS WILL NOT BE CONSIDERED FOR THIS NEED (Block scheduling etc)
- MUST BE ABLE TO WORK INDEPENDENTLY! Tech may be the only person on during the shift
- Traveler must have their own vehicle for this need.

- Work History
- Skills Checklist (pertinent to position) current to 1 year
- 1 Supervisory Signed Reference (from within the past two years of employment)
- License/Certifications Must be uploaded with profile
- BLS (AHA)
