Job Description
We are seeking a compassionate and skilled individual to join our team as a Physical Therapist in Home Health. This key role focuses on providing rehabilitative care to patients within the comfort of their own homes aiming to enhance their physical independence and quality of life.
Key Responsibilities of a Physical Therapist in Home Health
- Conduct comprehensive physical therapy assessments for home health patients to develop personalized treatment plans.
- Implement therapeutic interventions designed to improve mobility strength balance and coordination.
- Educate patients and caregivers on exercise programs and techniques to encourage patient independence.
- Collaborate with healthcare professionals to ensure a holistic approach to patient care including physicians and multidisciplinary team members.
- Document patient progress accurately adjusting treatment plans as necessary to align with patient goals and progress.
Schedule and Shift Details
This is a fulltime position with a flexible schedule primarily working weekdays with occasional availability for weekends based on patient needs.
Qualifications and Requirements
- Valid Physical Therapist (PT) license in the state of employment.
- Minimum of 1 year of experience in home health care or a related setting preferred.
- American Physical Therapy Association (APTA) membership is a plus.
- Ability to travel to patient homes; must have reliable transportation.
- Excellent communication and interpersonal skills for effective interaction with patients and their families.
