Utilizes assessment data to identify patient care needs to include patient education and discharge planning.
Recognizes and reports abnormal assessment data and incorporates findings into the plan of care: validates and revises expected outcomes based on assessment.
Accurately and legibly documents all patient care activities including but not limited to wound assessment patient care teaching demonstrated progress toward goals medication administration nursing flow sheets and all assessment forms.
Delegates tasks to the health care team members appropriately.
