Overview
The Home Care Nurse plays a crucial role in providing highquality healthcare to individuals in their own homes. This position is essential in ensuring that patients receive personalized care tailored to their specific needs facilitating a comfortable and familiar recovery environment. Home Care Nurses work closely with patients facing chronic illnesses recovering from surgery or requiring assistance with daily living activities. They are responsible for monitoring patients health status administering medications and collaborating with other healthcare professionals to create and implement effective care plans. Key Responsibilities
- Conduct initial patient assessments to evaluate health needs.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Administer medications and injections as prescribed monitoring for side effects.
- Perform wound care catheterization and other nursing procedures as needed.
- Monitor and document patients vital signs and health status regularly.
- Assist patients with daily living activities such as bathing and dressing.
- Provide education to patients and their families on health management and disease prevention.

Required Qualifications
- Current and valid nursing license in the state of practice.
- Bachelor s degree in nursing (BSC) preferred or ANM/GNM.
- Minimum of 1 year of nursing experience preferably in home health or community settings.
- Knowledge of home healthcare services and community resources.
- Excellent assessment critical thinking and problemsolving skills.
- Strong interpersonal and communication abilities.
- Ability to work independently and manage time effectively.
- Proficiency in using electronic health record (EHR) systems.
- Basic life support (BLS) certification required; advanced certifications preferred.
- Familiarity with patient care technologies and equipment.
