Job Description
Job Title: Physical Therapist Outpatient
Location: Concord California
Shift: Monday to Friday 8:00 AM 5:00 PM
Shifts Per Week: 5
Scheduled Hours: 40 hours per week
Salary Range: $85000 $95000 annually
Bonuses:
- Up to $20000 for relocation signon bonuses and/or student loan assistance
Job Overview:
This fulltime role provides an excellent opportunity to deliver quality patient care working in a supportive environment with a focus on rehabilitation. The position offers a standard Monday through Friday workweek allowing you to maintain a healthy worklife balance while treating an average of 14 patients per day.
Key Responsibilities:
- Perform comprehensive evaluations to assess patients functional needs and limitations.
- Develop and implement personalized treatment plans aimed at restoring function improving mobility and alleviating pain.
- Educate patients and their families on therapy exercises and recovery processes.
- Document patient progress and maintain accurate records in compliance with standards.
- Collaborate with other healthcare professionals to ensure coordinated care.
- Provide mentorship and guidance to support staff and interns as needed.
- Stay current with continuing education and best practices to deliver evidencebased care.

Qualifications:
- Education: Graduate of an accredited Physical Therapy program.
- Experience: New graduates are welcome to apply but 1 year of clinical experience is preferred.
- Licensure/Certification:
- Physical Therapist (PT) license in State or eligibility for licensure.
- Basic Life Support (BLS) certification or willingness to obtain within 30 days of hire.
