Job Description
Facility: North Memorial Health Maple Grove Hospital
Address: 9875 Hospital Dr Maple Grove MN 55369
Contract: 13 weeks
Shift: 8:00 AM 4:30 PM (40 Hour Guarantee)
Rate: $72/hr (MSW or CSW) OR $65/hr (BSW)
Job Description
The Social Worker provides services to customers and their families/significant others to help them adjust to illness injury or hospitalization. Assists the health team in understanding the significance of social economic emotional and family factors in relation to the customers illness treatment recovery or dying processes. Responsible for facilitating timely efficient and appropriate discharge of customers and coordination with next level of care including transmission of appropriate plan of care. Identifies needed community services and coordinates appropriate services for customers in need.
Requirement description :
- 2 Years Hospital Social Working Experience Required
- Acute Care (Hospital) Discharge Experience Required
- Adult and Geriatric Experience Required
- MSW or CSW Would accept a BSW at the decreased rate Required
- Epic Experience Preferred
ADDITIONAL REQUIREMENTS :
Weekend Requirements: Every 3rd Weekend
OnCall Requirements: No OnCall
Dress Code: Business Casual
