Job Description
Position Name: Licensed Social Worker Hospital
Location: Maple Grove MN 55369
Start Date: ASAP
Employment Type: Fulltime Travel Contract Temporary
Contract Length: 13 weeks (Travel Candidates Only)
Pay Rate: $2600 to $2640 Weekly Gross Pay (Nontaxable travel/housing and M&I stipend available for eligible travelers)
Unit: Acute Care Units
Shift: 8 Hours; Days (8:00 AM 4:30 PM)
Schedule: 40 hours/week guaranteed 5 shifts per week (MondayFriday) every 3rd weekend
- Current Social Work Experience: 2 years Required
- Hospital Social Work Experience: 2 years Required
- Hospital Discharge Facilitation Experience Required
- Adult and Geriatric Population Experience Required
- Prior Travel Experience Highly Preferred
- Degree in Social Work Required
- LMSW or LCSW or equivalent Social Worker License for the State of MN Required
- LBSW License for the State of MN (Accepted at a reduced rate)
- BLS Certification from AHA Required
The Social Worker provides services to customers and their families/significant others to help them adjust to illness injury or hospitalization.
- Assists the healthcare team in understanding the significance of social economic emotional and family factors in relation to the customers illness treatment recovery or endoflife processes.
- Facilitates timely efficient and appropriate discharge of customers and coordinates the transition to the next level of care ensuring proper transmission of the plan of care.
- Identifies needed community services and coordinates these services for customers requiring assistance.
