Job Description
Hiring Social Worker (LMSW)
Specialty: Hospital
Location: Maple Grove MN
Start: 1/6/2025
Duration: 13 Weeks
Shift Type: Days(8:00 AM 4:30 PM)
Pay: $3050.00 $3150.00 per week
The Social Worker provides services to customers and their families/significant others to help them adjust to illness injury or hospitalization. Assists the health team in understanding the significance of social economic emotional and family factors in relation to the customers illness treatment recovery or dying processes. Responsible for facilitating timely efficient and appropriate discharge of customers and coordination with next level of care including transmission of appropriate plan of care. Identifies needed community services and coordinates appropriate services for customers in need.
Requirements:
- 2 Years of Hospital Social Working Experience Required
- Acute Care (Hospital) Discharge Experience Required
- Adult and Geriatric Experience Required
- MSW or CSW Would accept a BSW $2870/Week
Certification Requirements:
- BLS from AHA
