The Role
We are seeking a detailoriented and proactive Intake/Referral Coordinator to join our team. In this vital role you will serve as the initial point of contact for new referrals ensuring seamless transitions into our care. Your work will help us connect patients to the services they need contributing to the quality care and support that We Care Home Health is known for.
What Youll Be Doing
- Receive evaluate and process all incoming referrals with accuracy and efficiency.
- Complete referral documentation including medical and demographic information.
- Verify insurance coverage and obtain prior authorizations from thirdparty payers.
- Coordinate with clinical and scheduling teams to ensure timely initiation of care.
- Act as a resource for families clients and referral sources providing information on available services and coverage options.
- Maintain accurate logs files and records to ensure compliance and followup on unresolved coverage issues.
- Collaborate with internal teams including marketing clinical and accounting staff to educate on insurance benefits resources and community support.
- Participate in quality improvement activities to enhance the intake/referral process.
What Makes You a Great Fit
- Minimum of one year of experience in intake referrals insurance authorizations or a related role preferred.
- Familiarity with community resources Medicare/state regulations and thirdparty/private payer reimbursement criteria.
- Excellent communication skills (verbal and written) with a focus on professionalism and teamwork.
- Strong attention to detail critical thinking and time management skills.
- Ability to work independently while managing multiple tasks efficiently.
- Proficiency in computer data entry and documentation.
