Temp - PT - Rehab Days Gladwin MI

Facility Specific Requirement: Cannot accept local candidates that live within 50 miles of the facility.

This position delegates responsibility to the Physical Therapist Assistant and Aides. Activities of this position are patientfocused and utilize assessment planning intervention and evaluation in accordance with physical therapy standards physician orders and governing policies and procedures.
(25%)* Evaluates the patient s function by performing formal/informal testing measures to determine the skeletal neurological respiratory and cardiovascular status.
(25%)* Develops a goal specific treatment regime based on assessment data and the patient s tolerance to carryout the treatment plan as prescribed.
(25%)* Submits a written treatment regime to be included as part of the plan of treatment authorized by and coordinated with the physician. Updates plan of care as needed to reflect changes in patient status. Cosigns verbal orders received by physical therapist assistant as necessary.
(25%)* Devises a schedule of treatment which is frequency and duration specific to be discussed with patient/family/caregiver and coordinated with clinical support staff.

• Inpatient & Outpatient Experience Required
• MI License must be active
• BLS (AHA) Required
• **WILL TAKE NEW GRADS**

• Work History
• Skills Checklist current to 1 year pertinent to the specialty
• 2 Supervisory Reference Forms taken within the last two years
• Candidates Permanent Address (Facility will not consider local candidates 50 mile radius restriction)
