Job Description
Nazareth Hospital is looking for a Family Medicine Physician to join their team in Philadelphia Pennsylvania. Trinity Health MidAtlantic is a diverse integrated system providing health services including five acute care hospitals three home health organizations wellness centers physician practices and three federal PACE programs.
Job Description:
- Fulltime position within 2701 Holmes Avenue Physicians Office Building at Nazareth Hospital
- 1:7 call including some weekends
- Provide direct services through comprehensive evaluation diagnosis treatment planning and treatment
- Provide procedural services within the scope of specialty training and clinical competence
- Communicate appropriately following confidentiality guidelines with internal and external providers to ensure quality and continuity of care Participate in the education of health professions students assigned to site rotations
- Medical record documentation will be in compliance with policies and procedures.
- Maintain an active Pennsylvania physician license board certification in family medicine specialty and controlled substance registration with the Drug Enforcement Administration. Abide by the credentialing and recredentialing policy.
- Maintain participation in Medicare Medicaid and other third party payer programs as required by administration.
- Management of chronic diseases and commonly encountered outpatient acute medical conditions.
- Knowledge and competencies in understanding and collaborating with the region s behavioral health and social service system.
- Comfort providing above services to individuals with severe/persistent mental illness substance use disorders and housing instability.
- Not accepting Locums candidates
- Not accepting candidates who have lapsed BC and need to obtain again
- Not accepting visa candidates
