Job Description
Location: Johnson City NY
Unit: LongTerm Care
Position Overview:
1. Degree Required: LPN
2. Specialty: LTCLPN
3. Shift Availability: Evenings Nights (Fulltime)
4. Start Date: 11/17/2024
5. Assignment Length: 26 weeks
6. Employment Type: Per Diem
Facility Information:
1. Facility Size: 160 beds
Shift Details:
1. Day Shift: 2:45 PM 11:00 PM (weekends only)
Pay & Compensation:
1. Estimated Pay Rate: $42 per hour
Orientation:
1. Length: 2 days
2. Days/Time: Thursday and Friday 9 am approximately 2 pm
Minimum Requirements:
1. Experience: 6 months of prior LPN experience required
2. Licensure: Active LPN license in good standing
3. Other Requirements: Resume primary verification 2 references skills license 2 IDs Immunizations CPR Cert required upon submission
Patient Care Requirements:
1. Responsibilities: Provide quality care to assigned patients including medication and treatment administration patient monitoring and documentation of care
2. Note: Easytomanage patient load but high autonomy expected
