Key Responsibilities
In this role you will work closely with healthcare professionals to support patients in managing their medication therapies optimize processes and provide critical assistance that positively impacts patient care.
Medication Review and Management
Thoroughly assess and document patients medication histories ensuring accuracy and completeness.
Collaborate with pharmacists and healthcare teams to address and resolve any discrepancies in medication records.
Patient Education and Assistance
Guide patients in understanding their medications including proper use side effects and adherence tips.
Provide answers to patient inquiries or direct them to the appropriate healthcare provider for further clarification.
Monitoring Medication Adherence
Use electronic health records (EHR) and related tools to track patient compliance with prescribed medication regimens.
Reach out to patients who may be struggling with adherence and communicate concerns to the healthcare team.
Care Coordination and Transitions
Support patients during transitions of care ensuring smooth medication management when patients are discharged or transferred to new care environments.
Manage medication refills prior authorizations and delivery services to ensure uninterrupted patient care.
Data Entry and Documentation
Accurately update and maintain patient records within the EHR system.
Document all patient interactions medication adjustments and communications with care providers.
Collaboration with Healthcare Teams
Work alongside pharmacists to identify potential drug interactions and contraindications.
Provide valuable data and insights to help refine and enhance patient care plans.
Regulatory Compliance and Quality Assurance
Ensure all activities comply with HIPAA state pharmacy regulations and other relevant healthcare standards.
Participate in continuous quality improvement efforts aimed at enhancing patient safety and medication outcomes.

Qualifications
Certification: Valid Pharmacy Technician Certification (CPhT) from an accredited certifying body (e.g. PTCB NHA).
Experience: Previous experience in a pharmacy setting ideally within care management or related fields.
Knowledge: Solid understanding of pharmacology medication therapy management (MTM) and chronic disease management.
Technical Skills: Proficiency with EHR systems pharmacy software and telehealth platforms.
Communication: Strong verbal and written communication abilities to facilitate effective interactions with patients and providers.
Attention to Detail: Exceptional accuracy in documenting medication orders and patient information.
ProblemSolving: Skilled at identifying medicationrelated issues and collaborating with others to resolve them effectively.
