Job Description
Position: Clinic Care Coordinator RN
Location: Fort Defiance AZ
Pay scale: $2756 $2856/week
Shift: Day
Responsibilities:
- Assess patients healthcare needs and develop individualized care plans.
- Coordinate and manage patient care across various healthcare settings including hospitals home care and rehabilitation.
- Collaborate with doctors nurses and other healthcare professionals to ensure comprehensive patient care.
- Monitor patient progress and adjust care plans as needed.
- Educate patients and their families about health conditions treatments and care plans.
Requirements:
- Required: BLS RN license
- Minimum 3 years of experience
