Job Description:
We are seeking a highly skilled and compassionate Primary Care Physician to join our team in our inclusive care for the elderly team in La Quinta CA. As a Primary Care Physician you will be responsible for providing primary care services to senior patients. You will work closely with our team of healthcare professionals to develop and implement treatment plans that address the physical emotional and social needs of our patients.
Responsibilities:
Conduct comprehensive physical examinations and medical histories for patients
Diagnose and treat acute and chronic illnesses injuries and diseases
Prescribe medications and provide ongoing medication management
Provide assessment planning primary care and continuous medical coverage to participants
Order and interpret diagnostic tests and screenings
Collaborate with other healthcare professionals to develop and implement treatment plans including communitybased clinicians to plan discharge from acute or longterm settings
Educate patients and their families on disease prevention and healthy lifestyle choices
Maintain accurate and uptodate patient records
Take part in rotating night and weekend oncall
Participate in quality improvement initiatives and adhere to all healthcare regulations and standards
Continuously stay updated on medical advancements and best practices in primary care
Requirements:
Medical degree from an accredited medical school
Valid and current state medical license
Board certification in family medicine or internal medicine
Minimum of 2 years of experience in primary care
Successful completion of residency accredited by the Accreditation Council for Graduate Medical Education
Active CA Physician License without restrictions
Board Certified Internal Medicine / Family Practice or Geriatric Medicine (preferred)
Active state CDS Certification and DEA
Currently enrolled or eligible to provide services to Medicare and MedoCal patients
Excellent communication and interpersonal skills
Ability to work collaboratively in a team environment
Strong organizational and time management skills
Compassionate and patientcentered approach to healthcare
Experience with electronic medical records systems is preferred
