Job Description
Job Title: Home Health Physical Therapist
Pay: $2602.00 per week
Contract Duration: 13 weeks
Experience Required: 3 years preferably in home health but strong skill nursing facility experience is also considered
Certifications Required: Basic Life Support
Licensure: MT PT License required
Travel: Mileage radius is about 30 miles with average daily mileage about 40
Billing: All inclusive rate
Hiring Process: HWL prescreen with manager direct interview and offer
Restrictions: 200 mile radius rule all requests for time off or schedule changes must be made at time of submission
Submittal Requirements: Address email full date of birth social security number and full name must be provided at time of submittal. No confirmations will be made without this information.
