Home Health Physical Therapist
Job Description
We are seeking a highly motivated and compassionate Home Health Physical Therapist to join our team. The ideal candidate will possess a strong background in physical therapy a passion for helping patients and a commitment to delivering exceptional care.
Key Responsibilities
- Provide comprehensive physical therapy services to patients in their homes.
- Develop individualized treatment plans tailored to each patients unique needs.
- Educate patients and their families on appropriate exercises and techniques.
- Collaborate with other healthcare professionals to ensure coordinated and effective care.
- Document patient progress and outcomes meticulously.
- Maintain accurate and uptodate patient records.
- Adhere to all relevant regulations and professional standards.
- Masters degree in Physical Therapy from an accredited university.
- Current license to practice physical therapy.
- Minimum of 2 years of experience as a Home Health Physical Therapist.
- Strong clinical skills and a deep understanding of physical therapy principles.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Demonstrated commitment to patientcentered care.
