Job Description
We are looking for a highly motivated and compassionate Home Health Physical Therapist to join our team. The ideal candidate will have a strong background in physical therapy a passion for helping patients and a commitment to providing exceptional care.
**Responsibilities:**

Provide comprehensive physical therapy services to homebound patients.

Develop individualized treatment plans based on each patients specific needs.

Educate patients and their families on proper exercises and techniques.

Collaborate with other healthcare professionals to ensure coordinated care.

Document patient progress and outcomes.

Maintain accurate and uptodate patient records.

Comply with all applicable regulations and standards.
**Qualifications:**

Masters degree in Physical Therapy from an accredited university.

Current license to practice physical therapy.

Minimum of 2 years of experience as a Home Health Physical Therapist.

Strong clinical skills and a thorough understanding of physical therapy principles.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a team.
