Job Description
Position: RN Case Manager
Location: Tuba City AZ
Pay scale: $2742 $2842/week
Shift: Days
Responsibilities:
- Assessing patients healthcare needs developing care plans and ensuring appropriate medical care is provided.
- Coordinating patient care across different healthcare providers and settings (e.g. hospitals outpatient clinics home care).
- Serving as a liaison between patients families healthcare providers and insurance companies.
- Monitoring patients progress evaluating outcomes and adjusting care plans as needed.
- Educating patients and their families about medical conditions treatments and selfcare management.
Requirements:
- Required: BLS RN license
- Utilization Review experience required
- Minimum 2 years of experience
