Job Description
Social Worker (LCSW) under general supervision responsible for providing clinical/case management services to patients and their families in order to promote the optimal individual interpersonal and social functioning of the patient and assure reintegration in to the community or the environment most suited to the patients continued wellbeing.
Responsibilities
- Conduct comprehensive patient assessments to determine mental health needs and develop individualized treatment plans.
- Provide behavioral therapy and clinical counseling to clients utilizing techniques such as cognitive behavioral therapy and motivational interviewing.
- Maintain accurate medical documentation and adhere to HIPAA regulations.
- Collaborate with multidisciplinary teams to ensure holistic patient care and support.
- Engage in crisis intervention and provide support for individuals with PTSD and developmental disabilities.
- Facilitate family counseling sessions and offer guidance in home and community care settings.
Qualifications
- Masters degree in Clinical Social Work from an accredited graduate school of Social Work or Masters degree in counseling or other behavioral health discipline REQUIRED
- Social social worker must be registered on the roster of psychotherapists who are nonlicensed and noncertified with the State of Vermont Office of Professional Regulation prior to employment.
- Must accept a flexible 40hour work week scheduled to meet the needs of patients and families within the guidelines of the bargaining agreement REQUIRED
- Experience with children and adolescentsREQUIRED
- One to two years of inpatient social work experience is preferred
